25-Gauge Vitrectomy for the Treatment of Massive Submacular Hemorrhage

An 82-year-old Caucasian female on systemic coumadin therapy with exudative AMD developed vitreous hemorrhage and massive hemorrhagic retinal detachment with light perception (LP) visual acuity. Twenty-five-gauge vitrectomy was performed and tPA 25 µg /0.1cc was injected subretinally. Vision improved to 20/400 1 month postoperatively.
