A 71-year-old high myope with a Boston K-Pro, placed due to complications of a prior chemical injury, presented with vision loss from a posterior RD. The causative break was posteriorly located with significant tractional membranes over a staphyloma.

The case presented several challenges, including cicatricial changes of the ocular surface, loss of external anatomic landmarks to locate the pars plana and need for both peripheral and macular maneuvers through the small aperture of the prosthetic.
